    Title of Book, Report, Article etc.
    Publication Date
    Expected date of publication
    For Sale
    Select appropriately. The answer is YES even if you are charging recovery costs. Explain this in Additional Information.
    Territory
    UK, Europe, North America, World etc.
    Period of Use
    For books and magazines - standard term is one print run. For television, video, DVD and web, the term is usually a period of time.
    Date Required
    Select Appropriately
    Licence to be made in favour of
    (optional) If purchase is for your own use simply leave blank. If you are licensing for another party e.g. an author clearing rights for a publisher, type in the name that the licence should be make in favour of.
